Health Information Exchange Opt-Out

Anderson participates in the exchange of health information, commonly known as Health Information Exchange (HIE). This participation is a way of allowing your health information to be shared by participating medical groups, hospitals, labs, other health care providers, health plans, and other authorized users through secure, electronic means. The purpose of the exchange is to give your health care providers, health plan, and other authorized recipients the ability to efficiently access medical information necessary for your treatment, payment for your care, and other lawful purposes. Your participation in the HIE is voluntary and subject to your right to opt-out. If you would like to opt-out of the HIE please complete the Health Information Exchange Opt-Out form.

If you wish to revoke a prior opt-out form, please complete the Health Information Exchange Cancellation of Prior Opt-Out. Send the completed form via postal mail or hand deliver to Anderson Hospital at:
